Arias was removed from the field on a cart during the third inning of Sunday's game against the Cardinals due to an apparent left ankle injury, Chris Assenheimer of the Elyria Chronicle-Telegram reports.
Arias was in pursuit of a Masyn Winn grounder when his cleat got stuck in the dirt, causing his lower leg to contort. The Guardians will send Arias for further testing before providing a diagnosis, but the infielder could be headed for a lengthy absence. Daniel Schneemann came off the bench to replace Arias at shortstop.
