The Phillies claimed Hoffmann (undisclosed) off waivers from the Diamondbacks on Thursday and optioned him to Triple-A Lehigh Valley.
Hoffmann has not made an appearance since early June because of an undisclosed injury suffered at the Triple-A level, but the Phillies felt comfortable enough with the reliever's health to scoop him up via waivers. When Hoffmann is ready to pitch, he'll provide relief depth at Lehigh Valley.

The Diamondbacks designated Hoffmann (undisclosed) for assignment Monday.
He'll give up his spot on the 40-man roster to right-hander Justin Martinez (elbow), who was reinstated from the 60-day injured list in a corresponding move. After winning a spot in Arizona's Opening Day bullpen coming out of spring training, Hoffmann was demoted to Triple-A Reno after submitting a 7.71 ERA and 2.06 WHIP in 11.2 innings out of the bullpen. He hasn't made an appearance for Reno since June 5 due to an unspecified injury and moved to the Triple-A club's 60-day IL over the All-Star break.

Triple-A Reno transferred Hoffmann from its 7-day injured list to the 60-day IL on Thursday while he recovers from an undisclosed injury.
After the Diamondbacks optioned Hoffmann to Triple-A on April 29, he submitted a 6.07 ERA and 1.95 WHIP across 13.1 innings over 12 relief appearances for Reno before landing on the shelf. With his move to the 60-day IL, Hoffmann won't be eligible to return to action for Reno until early August.

The Diamondbacks optioned Hoffmann to Triple-A Reno on Wednesday.
Hoffmann will go back down to the minors following his rough performance against the Brewers on Tuesday, when he recorded just one out while allowing eight runs (seven earned) on eight hits and a walk. Prior to that disastrous outing, Hoffman posted a 2.38 ERA, 1.32 WHIP and 12:7 K:BB across 11.1 major-league innings this year. Philip Abner was recalled from Triple-A in a corresponding move.

Hoffmann gave up one run on two hits and one walk while striking out three in 2.1 innings Sunday against the Blue Jays.
The one run allowed came on a Kazuma Okamoto home run. Hoffmann was shifted between the majors and minors late last year to little success, posting a 6.00 ERA and 2.00 WHIP in 11 appearances (12.1 innings), but he's turned it around to the tune of a 1-0 record with a 1.74 ERA in six games so far in 2026. The 26-year-old broke camp after an excellent spring where he earned a 2.25 ERA and 10 strikeouts across eight innings while improving his slider. The right-hander is looking to get more high-leverage opportunities with A.J. Puk (elbow) and Justin Martinez (UCL) starting on the injured list.

Royals' Andrew Hoffmann: Serving as 27th man for twin bill

The Royals optioned Hoffmann to Triple-A Omaha and then added him as the 27th man for Thursday's doubleheader versus the Cardinals.
Hoffmann will stick around with the Royals for the twin bill, but he was optioned for the purpose of opening a spot on the 26-man active roster for righty Thomas Hatch, who was called up from Triple-A. Since receiving his first MLB promotion last Wednesday, Hoffmann has made just one appearance out of the Kansas City bullpen, giving up two earned runs while striking out four batters over two innings.
