  • Jose Alvarado RP | PHI

Phillies' Jose Alvarado: Starts up rehab assignment

Alvarado (shoulder) made his first rehab appearance for Triple-A Lehigh Valley on Tuesday, tossing a no-hit inning while striking out one and walking one.

Alvarado was placed on the 10-day injured list Aug. 11 with left shoulder inflammation, but he appears to be on the cusp of a return. He may need only one or two more tune-up appearances at Triple-A before rejoining the Philadelphia bullpen.

  • Reiss Knehr RP | SD

Padres' Reiss Knehr: Demoted to minors

The Padres optioned Knehr to Triple-A El Paso on Wednesday.

The righty pitched four times in his latest MLB stint, allowing five runs on seven hits and six walks in 7.1 innings. Knehr has more walks (15) than strikeouts (14) in 20 big-league innings in 2021, and he has been pretty unimpressive overall, but the Padres' pitching woes and injuries could bring him back to San Diego before the season ends.

  • Dodgers' Victor Gonzalez: Sent to minors

The Dodgers optioned Gonzalez to Triple-A Oklahoma City on Wednesday, Fabian Ardaya of The Athletic reports.

The lefty has thrown 2.1 scoreless frames since returning from a knee injury Thursday. Gonzalez has had a solid season, posting a 3.57 ERA and 33:19 K:BB in 35.1 innings, but he was deemed not worthy of a roster spot now that Joe Kelly (undisclosed) is back from the injured list. Any injury in the Dodgers' bullpen would likely bring Gonzalez back to the big leagues.

  • Joe Kelly RP | LAD

Dodgers' Joe Kelly: Back from IL

Kelly (undisclosed) was activated from the COVID-19 injured list Wednesday, Fabian Ardaya of The Athletic reports.

Kelly had been on the virus-related IL since Aug. 10, but he returns to take Victor Gonzalez's spot on the active roster and in the bullpen. The righty was pitching well before going on the shelf, allowing only one run on two hits and a walk in his last 6.2 innings while striking out seven.

  • Tanner Rainey RP | DET

Nationals' Tanner Rainey: Remains shut down

Nationals manager Dave Martinez said Wednesday that Rainey (side) has yet to resume baseball activities at Triple-A Rochester, Mark Zuckerman of MASNSports.com reports.

After being optioned to Rochester on Aug. 12 to work on his mechanics due to his poor results in the big leagues, Rainey didn't appear in any games for the affiliate before being shut down last week with left side soreness. Martinez is optimistic that Rainey will move past the injury and pitch again in 2021, but since the right-hander will need some time to ramp up once he's cleared to resume activities, he may be multiple weeks away from game action.

  • Tejay Antone RP | CIN

Reds' Tejay Antone: Back on IL with forearm strain

The Reds placed Antone on the 10-day injured list Wednesday with a right forearm strain.

Antone had missed the past two months with a strained forearm before being activated from the 10-day IL Tuesday, but he lasted just one appearance before aggravating the injury. While the Reds haven't commented on the severity of Antone's setback, he'll almost certainly miss more than the minimum 10 days, and he could be in danger of sitting out the rest of the season. With Antone out of the picture for the time being, Mychal Givens, Michael Lorenzen, Lucas Sims and Amir Garrett look to be the top late-inning options for manager David Bell.

  • Bryse Wilson RP | MIL

Pirates' Bryse Wilson: Activated, starting Wednesday

The Pirates reinstated Wilson (arm) from the 10-day injured list Wednesday. He'll start for Pittsburgh in its series finale with Arizona.

Though Wilson spent the minimum amount of time on the IL due to right arm fatigue, manager Derek Shelton suggested the 23-year-old could be on a controlled pitch count in Wednesday's outing, according to Kevin Gorman of TribLive.com. Before landing on the IL, Wilson made three starts for Pittsburgh, going 0-2 with a 4.80 ERA and 1.20 WHIP.

  • Dodgers' Garrett Cleavinger: No timeline for return

Cleavinger (oblique) continues to conduct his rehab program at the Dodgers' spring training facility in Arizona, but manager Dave Roberts said the lefty reliever is "a ways away" from coming off the 10-day injured list, Eric Stephen of TrueBlueLA.com reports.

Cleavinger has been on the IL since Aug. 8 with a right oblique strain. Even before the injury surfaced, Cleavinger hadn't seen much use in high-leverage spots. Over his 22 appearances on the season, he's compiled a 3.00 ERA and 1.78 WHIP.

  • Dodgers' Scott Alexander: Rehabbing in Arizona

Alexander (shoulder) continues to conduct his rehab program at the Dodgers' spring training facility in Arizona, but manager Dave Roberts said the lefty reliever is "a ways away" from coming off the 10-day injured list, Eric Stephen of TrueBlueLA.com reports.

Stephen suggests Alexander could be a candidate to move to the 60-day IL, in which case he wouldn't be eligible to return until the back half of September. Though Alexander hasn't been shut down, the fact that the Dodgers aren't yet mapping out a rehab assignment for him suggests he's at least a couple weeks away from a return in a best-case scenario.

  • Blake Treinen RP | LAD

Dodgers' Blake Treinen: Increases holds lead

Treinen walked two batters and notched his only out via strikeout in a scoreless outing versus San Diego on Tuesday.

After Corey Knebel allowed two runs in the bottom of the eighth inning, Treinen was summoned to record the final out in the frame. He walked both Fernando Tatis Jr. and Manny Machado to bring the tying run to the plate but struck out Jake Cronenworth to end the threat. The right-hander leads MLB with 25 holds on the campaign while registering a 2.13 ERA and 10.6 K/9 across 55 innings.
