  The Phillies exercised Alvarado's (forearm) club option Wednesday. As expected, the Phillies have opted to pick up the $9.5 million club option on Alvarado for the 2026 season. He ended the 2025 campaign on the 15-day injured list due to a left forearm strain, and he missed 80 games earlier in the season due to a PED suspension. Across the 28 appearances he made in 2025, Alvarado posted a 3.81 ERA, 1.31 WHIP and seven saves over 26 innings. He'll have the offseason to heal from his injury and will aim to be available for the start of spring training.

• Phillies' Jose Alvarado: Expected back with Phillies in 2026

  Phillies president of baseball operations Dave Dombrowski said Thursday he would be "surprised" if Alvarado (forearm) is not back next season, Matt Gelb of The Athletic reports. The Phillies have a $9.5 million club option on Alvarado for 2026, and while Dombrowski did not confirm the option would be exercised, it seems that is the most likely scenario. Alvarado served an 80-game PED suspension and also missed time late in the year with a forearm injury, resulting in him appearing in only 28 games. The lefty remained effective when on the bump, posting a 3.81 ERA and 32:7 K:BB over 26 innings. It's not clear whether the forearm issue will affect Alvarado's offseason throwing, but Dombrowski did not mention any health concerns with the reliever moving forward.

• Phillies' Jose Alvarado: Won't pitch again this year

  Alvarado (forearm), who was placed on the 15-day injured list Friday, won't pitch again in 2025, Noah Levick of NBC Sports Philadelphia reports. Manager Rob Thomson confirmed prior to Friday's game against the Royals that Alvarado won't have enough time to recover from his strained left forearm before the regular season concludes. The 30-year-old left-hander is ineligible for the playoffs due to his PED suspension, which cost him 80 games earlier this year, as well. Alvarado had once worked his way into Philadelphia's closing mix due to the struggles of Jordan Romano (finger), but the former reverted back to a setup role upon his return in August due to the addition of Jhoan Duran. Alvarado will close the campaign with a 3.81 ERA, 1.31 WHIP, 32:7 K:BB and five holds while converting seven of his eight save chances. He has a $9 million club option for 2026.

  Major League Baseball suspended Alvarado for 80 games Sunday due to a positive test for a performance-enhancing substance, Todd Zolecki of MLB.com reports. Alvarado has been a central part of Philadelphia's bullpen this season, posting a 2.70 ERA, 1.20 WHIP and 25:4 K:BB over 20 innings while successfully converting all seven of his save chances. His suspension will now take him out of action until mid-August, and Alvarado won't be eligible to pitch in the postseason. Jordan Romano had already started getting more frequent save chances of late, and he'll likely receive even more ninth-inning opportunities with Alvarado out.

The southpaw needed just two pitches to get Xavier Edwards to line out to right field. Alvarado has been dominant to begin the season, going 5-for-5 in converting save chances while posting a 2.79 ERA, 1.34 WHIP and 14:2 K:BB through 9.2 innings. Then with the bases loaded and one out, a hit-by-pitch followed by a passed ball allowed two more runners to score, saddling Alvarado with his second blown save and third loss of the season. Sunday's outing came on the heels of five consecutive scoreless appearances since his last blown save, which came May 24 in Colorado.
