Jurgens entered the NFL's five-step concussion protocol Monday, Mike Garafolo of NFL Network reports.

Jurgens self-reported concussion symptoms Monday morning and was deemed to have sustained a concussion during the Eagles' Week 4 loss to the Rams. The starting left guard will now need to progress through the league's five-step protocol to return to full practice and be cleared to participate in Sunday's game against the Jaguars in London.

Westbrook-Ikhine reverted to the practice squad with Indianapolis on Monday, per the NFL's official transaction log.

Westbrook-Ikhine was elevated to the active roster ahead of Sunday's 30-13 win over the Commanders. The wide receiver failed to record any stats while playing 11 snaps on offense, and he added one tackle on kick coverage while logging 13 snaps with the special-teams unit. With Keenan Allen (groin) not 100 percent, it's possible that Westbrook-Ikhine is elevated again for the team's Week 5 matchup with Pittsburgh.

Boutte caught three of four targets for 36 yards in Sunday's 34-30 loss to the Cowboys.

With Nico Collins back from a two-game absence due to a hamstring injury, he dominated targets and left the remainder of the Houston pass catchers fighting for leftovers. Boutte ran 24 routes on C.J. Stroud's 35 dropbacks, but the summer trade pickup has yet to really make an impact with the Texans, recording nine catches for 115 yards on 13 targets after four games.

Smith (concussion) has been ruled out for the remainder of Monday night's game against the Saints, Tori McElhaney of the Falcons' official site reports.

Smith exited the contest in the first quarter after being down on the field for several minutes. An independent neurologist ruled that the outside linebacker had sustained a concussion, and he will not be allowed to return to the game. Smith will now enter the league's five-step protocol to return to full contact play.

Horton brought in his lone target for a six-yard touchdown during Sunday's 30-23 win over the Chargers.

Horton saw just one target from Sam Darnold in Sunday's game, but the second-year wideout answered the call by hauling in the pass for a six-yard touchdown early in the third quarter to extend Seattle's lead to 27-6. It was Horton's fifth touchdown catch of his young NFL career and first since his two-TD performance in Week 9 of the 2025 season, which was the last game of his rookie campaign due to shin and groin injuries. Horton is firmly entrenched as the Seahawks' WR4 on the roster behind Jaxon Smith-Njigba, Rashid Shaheed and Cooper Kupp, and Horton is also behind Shaheed as a returner on kickoffs and punts.

Haener reverted to the Giants' practice squad Monday, per the NFL's transaction log.

Haener was active for Sunday's 36-24 win versus the Cardinals, but his services were not needed, as veteran journeyman Jameis Winston started and finished the game at quarterback. Whether Haener is called up again in Week 5 might depend on how quickly recent trade acquisition J.J. McCarthy is able to familiarize himself with his new playbook.

The Jaguars signed Huzzie off the Texans' practice squad Monday, Aaron Wilson of KPRC 2 Houston reports.

The Jaguars stole Huzzie off the Texans' practice squad because Christian Braswell (knee) and Montaric Brown (hamstring) are suffering from injuries. The cornerback is set to be the last line of depth in case two of the other defensive backs ahead of him succumb to injury.

Tucker rushed three times for 10 yards and a touchdown in Sunday's 17-14 loss to the Packers.

Tucker played three offensive snaps and handled a carry on all of them, punching in a short two-yard touchdown in the second quarter. Tucker would likely be the biggest beneficiary in the event of a Bucky Irving injury, leaving Kenny Gainwell in a change-of-pace receiving role.

Perine rushed three times for seven yards in Sunday's 22-17 loss to the Jaguars.

Chase Brown carried the ball nine times for just 10 yards, and the Bengals rushed 15 times for just 23 yards as a team, with Joe Burrow scoring a rushing touchdown. Perine has rushed 13 times for 42 yards (3.2 YPC) through four games. He would need an injury to Brown to gain any fantasy relevance.

Allen turned three carries into one yard during the Commanders' 30-13 loss to the Colts on Sunday in London.

Allen's first carry came early in the first quarter, but the rookie sixth-rounder didn't touch the ball again until the last five minutes of the fourth period. Even with Rachaad White (shoulder) inactive, Allen was on the field for only 12 of the Commanders' 64 offensive snaps (18.8 percent), which was behind both Jacory Croskey-Merritt (29) and recent addition Austin Ekeler (17). White is expected to return to practice ahead of the Commanders' Week 5 game against the Giants, and if he's cleared to return, then Allen would seemingly move down to the RB4 spot on the roster behind Croskey-Merritt, White and Ekeler.
