    Pirates' Jose Hernandez: DFA'd by Pittsburgh

    The Pirates designated Hernandez for assignment Tuesday, Alex Stumpf of MLB.com reports.

    A week after being optioned to Triple-A Indianapolis, Hernandez will now lose his spot on the 40-man roster in favor of Dennis Santana, who was claimed off waivers Tuesday. The 26-year-old southpaw owns a 4.82 ERA through 56 career MLB innings, but he has not been nearly as successful this season in the minors, posting a 7.04 ERA and 1.85 WHIP through 15.1 frames.

    Cubs' Colten Brewer: Returns from IL

    The Cubs reinstated Brewer (back) from the injured list Tuesday, Maddie Lee of the Chicago Sun-Times reports.

    Since landing on the IL in mid-May, Brewer has appeared in three rehab games at Triple-A Iowa, allowing one earned run in four innings while striking out six batters and walking three. The 31-year-old owns a 3.09 ERA and 1.11 WHIP through 11.2 big-league frames this season and should return to a middle-relief role now that he's healthy. Ben Brown (neck) was placed on the injured list in a corresponding move.

    Cubs' Ben Brown: Hitting IL with neck strain

    Brown will be placed on the injured list with a left neck strain, Taylor McGregor of Marquee Sports Network reports.

    Brown has been dealing with the strain over the last few weeks and will head back to Chicago to undergo more testing. The severity of the injury is unclear but we should learn more in the coming days. Jordan Wicks is the primary candidate to take Brown's spot in the rotation, while Colten Brewer (back) should join the bullpen after being reinstated from the 15-day IL.

  • Cardinals' Giovanny Gallegos: Another outing on tap

    Gallegos (shoulder) will throw at least one inning of relief Tuesday at Double-A Springfield, Daniel Guerrero of the St. Louis Post-Dispatch reports.

    Gallegos is set for his fifth minor-league rehab appearance Tuesday and could toss multiple innings for the first time during his rehab stint. In four innings of minor-league work, Gallegos has allowed five runs on five hits and two walks. He should be approaching a return to the big-league roster.

    Cardinals' Riley O'Brien: Tossing live BP

    O'Brien (forearm) will throw a live batting practice session Tuesday at Double-A Springfield, Daniel Guerrero of the St. Louis Post-Dispatch reports.

    O'Brien will face live hitters for the first time since going on the injured list back in March. If the live session goes well, the 29-year-old could begin a rehab assignment later in the week.

    Phillies' Michael Rucker: Continuing rehab at Double-A

    Rucker's rehab assignment as he comes back from a right hand injury was transferred from Single-A Clearwater to Double-A Reading on Tuesday.

    Rucker made his first rehab appearance Friday with Clearwater, tossing a scoreless inning. The outing was Rucker's first in-game action since March 1, and he'll continue his recovery at Double-A Reading. Rucker will likely require a handful of rehab outings before being reinstated.

    Phillies' Luis Ortiz: Starting rehab stint

    Ortiz (ankle/shoulder) will begin a rehab assignment at Double-A Reading, the Phillies announced Tuesday.

    Ortiz was removed from his minor-league rehab assignment in late April after experiencing inflammation in his right shoulder. He was initially placed on the IL with a sprained left ankle, which is what he was rehabbing before the setback. It's unclear how long of an assignment he'll require, but it's an encouraging sign for the 28-year-old, who's been sidelined since March 30.

  • Kevin Herget RP | NYM

    Brewers' Kevin Herget: Sent to Triple-A

    Herget was optioned to Triple-A Nashville on Tuesday.

    Herget will make room on the active roster for Carlos F. Rodriguez, who'll start Tuesday's game against the Blue Jays. Herget owns a 2.08 ERA with a save across 8.2 innings with the Brewers this season.

    Mets' Edwin Diaz: Set to be activated Thursday

    Mets manager Carlos Mendoz said Tuesday that Diaz (shoulder) will be activated from the 15-day injured list Thursday, Will Sammon of The Athletic reports.

    Diaz is done with his rehab assignment after making two scoreless appearances with High-A Brooklyn. However, the Mets will wait a couple more days before returning the reliever to the active roster. Mendoza didn't divulge plans for Diaz's immediate role, but he should serve as the club's closer sooner rather than later. Diaz has been out since late May with a right shoulder impingement.

  • Yency Almonte RP | LAD

    Cubs' Yency Almonte: Completes bullpen session

    Almonte (shoulder) threw about 30 pitches during a bullpen session Monday, Meghan Montemurro of the Chicago Tribune reports.

    Almonte completed his first bullpen session since being cleared for mound work Friday. He's been rehabbing at the Cubs' facility in Arizona, where he'll remain before being shipped out on a rehab assignment. Almonte hasn't pitched since May 7 and is battling a strained throwing shoulder.
