Wells (head) has been sent to Triple-A Scranton/Wilkes-Barre to begin a rehab assignment.

Wells is ready to participate in games again after going on the 10-day injured list earlier this month with cervical headaches. He's eligible for activation now and hasn't missed much time, so Wells' rehab assignment should be brief.

Baltimore optioned Nunez to Triple-A Norfolk on Tuesday.

Nunez was last in action on Sunday, when he pitched one scoreless inning against San Diego. On the campaign, the right-hander has posted a 4.98 ERA, 1.28 WHIP and 38:14 K:BB through 34.1 innings. The 24-year-old will head down to Norfolk to make room for Ryan Helsley on the active roster.

The Orioles activated Helsley (elbow) from the 15-day injured list Tuesday.

Helsley wound up being sidelined for seven weeks with right elbow inflammation. He struck out five of seven batters faced over two scoreless rehab innings with Triple-A Norfolk and should slide right back into the closer role in Baltimore. Helsley posted a 2.53 ERA and 15:7 K:BB over 10.2 frames while notching seven saves before he got injured.

Sloan is not part of the Mariners' "immediate plans," Adam Jude of The Seattle Times reports.

Sloan's 4.44 ERA at Double-A Arkansas is a tad bloated thanks to a couple clunkers, but he boasts a 58:12 K:BB over 46.2 innings in his 11 starts. The 20-year-old right-hander is in the Mariners' plans for later this summer, per Jude, but Seattle already has six viable starting pitchers at the major-league level, and fellow prospect Kade Anderson could also be ahead of Sloan in the pecking order.

Anderson is not part of the Mariners' "immediate plans," Adam Jude of The Seattle Times reports.

Anderson's first 11 starts in pro ball couldn't have gone much better, as he's collected a microscopic 1.13 ERA, 0.67 WHIP and 82:7 K:BB over 55.2 innings with Double-A Arkansas. However, while the Mariners do expect Anderson to contribute in the majors at some point this season, a big-league debut for the top prospect is not imminent. Seattle already has six capable starting pitchers at the major-league level, complicating the timeline for Anderson's debut, but there could come a point when the southpaw forces the issue.

Sands underwent an MRI on Friday that showed healing in his injured forearm, though inflammation was still present, MLB.com reports.

The Twins placed Sands on the 15-day injured list May 2 due to what the team described as a mild right forearm strain, resulting in the reliever being shut down from throwing for a couple of weeks. He made enough progress to begin a minor-league rehab assignment June 2, only to be returned to Minnesota after his forearm bothered him during his lone appearance with Triple-A St. Paul. While Sands appears to have avoided any sort of major setback, the Twins will once again have him pause his throwing program to allow the inflammation to dissipate. He now appears unlikely to return from the IL until at least early July.

Smith will be shut down for two weeks after being diagnosed with a left shoulder impingement, James Fegan of SoxMachine.com reports.

Smith will require two weeks to get ramped back up following the shutdown period, so he's looking at four weeks away from games in a best-case scenario. The left-hander had seemed close to getting his first opportunity at the major-league level, but that will not happen anytime soon now. Smith has collected a 4.67 ERA and 77:36 K:BB over 52 innings covering 14 starts with Triple-A Charlotte in 2026.

Gasper (groin) remains out of the lineup for Tuesday's contest versus the Blue Jays.

He'll miss a second straight start after having started each of the previous seven tilts. Gasper is considered day-to-day. Connor Wong will do the catching and Masataka Yoshida will serve as the designated hitter for the Red Sox on Tuesday.

Triple-A St. Paul transferred Olivar from the 7-day injured list to the 60-day IL on June 4 while he recovers from a right knee sprain, Twins writer Brandon Warne reports.

The Twins haven't ruled Olivar out for the season, but he'll be on the shelf through at least the end of July while he recovers from the knee injury. After posting a .978 OPS at Double-A Wichita to begin the campaign, Olivar was promoted to Triple-A on May 20 and had slashed .259/.259/.556 with two home runs, two doubles and one stolen base in seven games for St. Paul before spraining his knee.

Triple-A St. Paul placed Mendez on its 7-day injured list Saturday due to a left shoulder impingement, Theodore Tollefson of ZoneCoverage.com reports.

After opening the season at Double-A Wichita, Mendez earned a promotion to Triple-A in early May and has continued mashing, producing a .321/.431/.468 slash line with four home runs and three stolen bases in 130 plate appearances for St. Paul. Assuming Mendez is able to move past the shoulder issue quickly, he'll have a shot at making his MLB debut at some point in the second half of the season.
